ὑπατείας Οὐϊρίου Νεπωτιανοῦ καὶ Τεττίου ΦακούνδουΦακούντου τῶν λαμπροτάτων. Φλαουΐῳ Ἰουλιανῷ διοικοῦντι συνδικίαν Ὀξυρυγχίτου παρὰ Αὐρηλίων Θεωνίνου καὶ Ἥρωνος καὶ Διδύμου καὶ Σιλβανοῦ δημοσίων ἰατρῶν τῆς αὐτῆς πόλεως. ἐπεστάλημεν ὑπὸ τῆς σῆς ἐμμελείας, ἐκ βιβλιδίων ἐπιδοθέντων ὑπὸ Αὐρηλίου Πτολεμαίου πρυτανεύσαντος τῶν αὐτόθι, ὥστε ἐπιθεω ρῆσαι τὸν τούτου γεωργὸν, Ἆπιν τοὔνομα, καὶ ἐγγράφωςἐνγράφως προσφωνῆσαι τὴν τούτου διά θεσιν. ὅθεν ἐπεθεωρήσαμεν αὐτὸν ἔχοντα ἐπὶ τοῦ δεξιοῦ μέρους τοῦ τραχήλου ἀμυχὰςαμχας καὶ ὠμοπλάτης πελιωμάτιονπελειωμάτιον καὶ ἐπὶ τοῦ δεξιοῦ ἀγκῶνοςἀνκῶνος ἀμυχὴν καὶ ἐπὶ τοῦ κάτω δεξιοῦ βλεφάρου πελιωμάτιονπεριωματιον, ὅπερ προσφωνοῦμεν. ὑπατείας τῆς προκειμένης Παχὼν ια.
